Tips for Accurately Answering the Task Questions #401 – 417 (Value-Based Care Questions)

1) Document what you did for your client
2) Document what you observed
3) Document what the client reported to you
Taking your time to answer these questions accurately will help your client get the best possible care.

404 | Has the client expressed feelings of loneliness or distress? | -When you asked the client, did they tell you they felt lonely, isolated, or neglected? -Is the client no longer doing activities they previously did? -Did you observe the client no longer interested in doing activities, less engaged in social events, or not interacting with family or friends? -Is the client withdrawn and no longer want visitors as they did previously? |
405 | Has the client received the influenza vaccine within the last year? | -Did the client, family member, or responsible caregiver tell you the client received the flu vaccine in the last year? |
410 | Has there been a change in the client’s skin condition today? | -Did the client tell you or did you observe any difference in skin integrity: a bruise, cut, blisters, black and blue, open sore, ulcer? -Did the client tell you or did you see a burn, injury, rash? Skin tears, rash, oozing? -Ask the client if the condition is getting worse. |

415 | Has the client experienced any increase in pain? | -Did the client tell you they are in more pain than usual? -Ask the client to rate the level of pain from 0-10 today compared to yesterday; has there been an increase in their rate of pain? -Did you notice non-verbal signs of pain that is not usual for the client? Wincing, crying, moaning, guarding, or rubbing the area. -Ask the client if this is a normal level of pain for them and if it is manageable (by treatments, comfort measures, and/or medications). -Ask if the client took their regular medications to manage their pain. |
