The PPE online order form is available in three languages: English, Spanish, and Chinese. Simply fill out some basic information about yourself, including your name, Personal Care ID number, mailing address, and email address, and then choose the items you need! After selecting the items, click “Submit”, and your request will be automatically submitted. You will receive an email confirmation of your order. One VERY IMPORTANT thing you can do right now is to review your emergency contact information with your supervisor!  Be sure your supervisor has your current mobile phone number and email address. If this information changes, be sure to let your supervisor know. We want to be able to contact you when emergencies or important incidents or events occur, so it is important that we have your correct and most up-to-date contact information on hand.

September 3, 2021 – In compliance with the recently passed New York State HERO Act, we are sharing with you VNS Health’s Airborne Infectious Disease Prevention Plan (click here to view). In accordance with this new law, VNS Health is required to devise a plan that is to be activated, implemented, and revised as needed, in the event the New York State Commissioner of Health declares an “outbreak” of any airborne infectious disease, including but not limited to COVID-19. The new law requires us to share this document with our staff. It will also be posted at all VNS Health offices. The COVID-19 pandemic has caused much anxiety on everyone.  If you’d like to talk to a mental health counselor, you can contact one through 1199. In case it’s helpful, here’s a brochure that was created by 1199 on mental health support. Here are some other helpful resources:
