Important: Answer the Task Questions Carefully When Clocking Out!

Dear Home Health Aide Team Member,
We appreciate your efforts to clock in and clock out successfully. Recently we have noticed a significant increase in the number of inaccurate responses to the Plan of Care task questions between the numbers of 401- 417. (These questions are also known as the Value-Based Care questions.) Carefully and accurately answering these questions on your client’s Plan of Care is very important for providing essential and timely health care to your client.

It is very important to mark these task questions with a checkmark for YES or X for NO when clocking out.
- Take your time when reading the task questions and make sure to read them thoroughly. Please don’t rush.
- Answer each of the Plan of Care task questions accurately.
Why are the task questions between #401-417 so important?
- These tasks are important because they help ensure the client gets the essential care they need. This drives the quality of care we provide to our clients and your responses allow us to take appropriate action.
- Your answers to these task questions alert Personal Care team members and Supervisors that there is something wrong or different with your client that requires immediate attention.
What happens if I click “yes-yes-yes” without really reading the questions when I clock out?
- Answering “yes” to tasks related to client’s change in condition means there is a negative situation occurring with your client.
- Answering “yes” means that your client requires immediate attention.
- Answering “yes” when the actual answer is “no” creates a false alert for your Supervisor.
- An increase in false alerts causes your Supervisor to spend time addressing false situations that are not urgent or needed.
- An increase in false alerts decreases your Supervisor’s ability to follow-up with real quality-of-care concerns for your client.
What happens if I click “no” to task #405: “Has the client received the influenza vaccine within the last year?”
- Answering “no” to this task means the client needs to follow-up on receiving the Flu vaccine.
Do I have to answer these questions #401-417 (Value Based Care questions) on every clock-out?
- Yes, you must answer these questions every time you clock out.
