Important Change to Your Client’s Plan of Care

Dear Team,

We would like to inform you of an important change that is being made to how tasks will be listed on your client’s Plan of Care in the HHAeXchange (HHAx) mobile app.

What’s changing: You will no longer see the full list of all available Plan of Care tasks as you have been seeing them. Instead, the only tasks you’ll see are those that are listed on your client’s Plan of Care. This important change will make it easier for you because it’ll help ensure that only those tasks that have been approved for your client are being performed each day.

Here are the 3 key things you need to know about this important change:

  1. If there is an active Plan of Care attached to your client’s master week, you will see this in the HHAx mobile app. You must respond to ALL tasks that are listed. Either select that the tasks were performed or completed… or that your client refused the task for that day.
  2. If NO PLAN OF CARE is listed, you will see all tasks listed under “Other tasks.” If this happens, you should use the paper Plan of Care that is in your client’s home to accurately select the tasks that have been completed and/or refused by your client that day. Please note, you only need to respond to the tasks on the paper Plan of Care. Please also be sure to call your supervisor to let them know that no Plan of Care is showing in the HHAx app for your client.
  3. If there is a task that needs to be performed for your client that is NOT listed on their Plan of Care, you must call your supervisor to request and explain why the change needs to be made to the Plan of Care. You will no longer be able to select from tasks that are not already on your client’s Plan of Care.
