Reminder: How to Order Personal Protective Equipment (PPE)

If you need to order PPE, please remember to use the online ordering form to place your order. The link can be found here. Your items will then be shipped to your home address!

Please note that going into a VNS Health office without placing an order for PPE is not recommended, as we do not keep an inventory of all PPE items in each office.

Note: For Westchester and Long Island, if you would like to pick up your PPE items in person, you must contact your supervisor to schedule an appointment.

How to Order PPE:

Simply fill out some basic information about yourself, including your name, Personal Care ID number, mailing address, and email address, and then choose the items you need! After selecting the items, click “Submit”, and your request will be sent to the supply team. You will receive an email confirmation of your order. The order form is available in three languages–English, Spanish, and Chinese.
