Please Read: Important VBP Questions Update When Clocking Out

Dear Team,
You will now be asked to answer some new questions about your client each day when you clock out of a client visit. These questions will help us – and you – to continue to provide the best possible care. Instructions for this new update are included below for clocking out on HHAx. We’ve also included instructions for manually clocking out on a client’s telephone.
Here are examples of some of the new questions you’ll need to answer:
- Has the client received the influenza vaccine within the last year?
- Has the client been injured as a result of a fall in the last 90 days?
- Has there been a change in the client’s skin condition?
- Has the client expressed feelings of loneliness or distress?

Please note: A question may not appear if we no longer need this information. (For instance, if a patient has received a flu shot, you won’t have to keep answering this question.)
If you’re clocking out using the HHAx App: These new questions will appear in the “Value Based Care Questions” section that’s under the “Plan of Care” task section that you see when you clock out for the day.
Just like the Plan of Care tasks that you are required to mark with a checkmark or X, the Value Based Care Questions that you must answer will have a red asterisk * next to them.

If you forget to answer the questions, don’t worry! – the HHAx app will remind you with a pop-up box.
Once you mark the Plan of Care tasks and answer the questions, you will be successfully clocked out!
