VNS Health Needs Your Help to Improve Patient Care!

Reminder: Please complete the Patient Safety Survey by Sunday, May 21st

Dear Team,

At VNS Health Personal Care, we firmly believe your role in caring for our patients is critically important.

The quality of care you provide and how you deliver that care to your patients is important to you and to us. You are our eyes and ears as frontline workers, and we believe you are in the best position to help us improve upon the care we provide as an organization. 

As an important member of the patient care team, we value your input. With this in mind, we are conducting a Patient Safety Survey so that we can gain a better understanding of the quality of our care in 2023.

You should have received this survey on Monday, May 1st. It came by email from a third party called the Center of Patient Safety. This online survey should take no more than 10 minutes for you to complete. It is available in English, Spanish, Russian, Haitian Creole, as well as in Simplified Chinese, and your responses are completely anonymous.

I ask that you complete the survey no later than Sunday, May 21st, 2023. I very much appreciate your participation in this extremely important effort. Your honest responses will help us to identify areas where we can improve the care we provide to our patients.

Please look for an email from the Center of Patient Safety. If you have not received the email, or if you have any questions, please call 646-369-4855 and someone can assist you.
