- Important reminder: If the Plan of Care is missing from either the HHAeXchange app or in the system when clocking out by your client’s telephone, make sure you call your supervisor to let them know! You should also follow these steps to successfully clock out:

Best practices for using a Hoyer lift for transferring patients
Hoyer lifts can help safely move a patient from one place to another (chair to bed or bed to bath). A lift can reduce risk of injury to patients and caregivers when properly used. To read all of the FDA’s tips for how to safely use Hoyer patient lifts, click here.
